In accordance with the Indiana Department of Health (IDOH) regulations, instructors at the FAB Training Academy are required to possess specific credentials to ensure the highest quality of education in eyelash extension training. Per 410 IAC 1-8-12, all instructors must have a minimum of two years of practical experience in applying eyelash extensions.Additionally, they must present evidence of their eyelash extension training, which includes submitting applicable licenses or certifications along with related curriculum or transcripts that display eyelash extension-specific subject matter. This ensures that our faculty are not only experienced practitioners but also have formal training aligned with state standards.

To ensure student readiness and compliance with state licensing eligibility, the following prerequisites are required for enrollment into the FAB Training Academy Eyelash Extension Program:

• Must be at least 18 years of age by the completion of the program or have  parent/guardian sign authorization for enrollment if under the age of 18.

• High school diploma or GED

• Valid government-issued photo identification

• Completed student enrollment application

• Signed student agreement and commitment form

These prerequisites are set to maintain program quality and ensure each participant is fully prepared to meet the rigors of the training, complete the IDOH-required hours, and pass certification for professional practice.

All applications must be submitted no later than 14 days prior to the scheduled class start date to be considered for enrollment in the FAB Training Academy Eyelash Extension Program.

Late or incomplete applications will not be accepted. Early submission is strongly encouraged due to limited class capacity.
