The program faculty holds a Masters in Health Administration.

1. Be at least 18 years old

& Possess a high school diploma or GED

& Hold an active CNA certification in good standing

& Pass background screening requirements

& Meet physical and clinical participation requirements

& Submit immunization documentation (if required)

& Must be at least 18 years of age.

1. Have proof of a high school diploma, or equivalent.
2. Be a certified nurse aide (CNA) in good standing in Indiana (no verified complaints or disciplinary action).
3. Work a minimum of 1,000 hours as a CNA within the past 24 months or request (and be granted) a waiver by the Certified Healthcare Professions Commission for a reduced work requirement.
4. Complete a state-approved QMA training program consisting of a minimum of 100 hours. Approved training programs can be found here.
5. Successfully pass the written competency evaluation.

Application deadlines are 4 weeks prior to class start dates on a rolling calender year.
