All faculty are Certified Community Health Workers with a Bachelors level or higher degree. They have also received their concentration in this Chronic Care Program.
Our program consists of 8 modules that include the following topics: An Overview of Chronic Health Conditions in Indiana Weight Management: Obesity, Nutrition, and Activity Tobacco Use & Evidence-Based Cessation Methods The Effects of SUD & OUD on Chronic Conditions Diabetes in Indiana & Its Effects on Population Health The Prevalence of Cardiovascular Disease: Causes, Effects, and Prevention Child Health in Indiana—Current Issues & Best Practices COVID and Chronic Conditions
They must be a high school graduate and over the age of 18.
