All faculty are Certified Community Health Workers with a high school diploma or higher. They have also received their concentration in this Chronic Care Program.
Our program consists of 11 modules that include the following topics: An Overview of Chronic Health Conditions in Indiana, Weight Management: Obesity, Nutrition, and Activity, Tobacco Use & Evidence-Based Cessation Methods, The Effects of SUD & OUD on Chronic Conditions, Diabetes in Indiana & Its Effects on Population Health, The Prevalence of Cardiovascular Disease: Causes, Effects, and Prevention, Child Health in Indiana—Current Issues & Best Practices, COVID, Emergency Preparedness & the CHW, Chronic Conditions in Under-represented Racial & Ethnic Minorities in Indiana, Effects of Social Determinants of Health on Chronic Conditions, and the Role of the CHW in Chronic Disease Prevention.
Must be at least 18 years or older for certificate. Must have at least a high school diploma or GED. Must have a personal email address (a computer and internet access is required).
